Just recently our office started getting a lot of calls from specialist offices asking for patients phone numbers because they are not listed on the referral form. Why this only recently started happening, I am not sure. I know the referral form only shows Home and Work numbers. A good portion of our patients only list a cell number. Wondering if anyone has a modified referral form (reforder.rpt) that includes cell phone. We are currently on the stand alone EMR version 9.8. I only have experience with VFE, not crystal reports. I have attempted to try and modify but apparently don't have the correct program to do so.

I don't have my referrals set up for a cell phone but my test form has it. You can take the formula and code and insert it into your referral report.

Hi Cliff,
I am wanting to insert Cell and Guarantor info into our tstorder form as well. I tried downloading your file, but it appears to be blank. Do you mind re-sharing?
