Wondering if anyone has an ASC NSQIP Risk Calculator form that they are willing to share. Trying not to "reinvent the wheel" if I don't have to.

the ASC Surgical Risk Calculator appears to be a web-based decision support tool that is dependent upon the surgery being performed and a host of other variables. The risk calculation algorithm performs some predictive modeling by referencing a database of over a million surgery outcomes and 21 separate risk factors (which is probably updated on a regular basis). In short, I doubt anyone would want to reproduce this tool's functionality in Centricity. Would it suffice to have an encounter form that provided the ability to launch the web-based calculator? That would be the safest and easiest thing to do in my opinion, but the resultant score would need to be manually entered on the form once it was calculated (assuming that's the information you are after). You could also set up a custom link in Centricity's navigation bar to go to the online calculator. Not sure if you have any familiarity with VFE, but you can also open up a web page by clicking on a button in a form. Would be happy to show you how to do that. Hope you find some useful information in my response.
I have already included the website navigation button in a VFE Form however was hoping that we would not have to print and scan the results into the chart, which is what our current workflow is. You are right, there are too many variables to easily develop a form, so I guess we will continue with scanning the printout from the website.

The only other thing that you might consider is printing to a pdf file. You can then attach the pdf file to the document in Centricity. This would save you the step of scanning it into the chart.
