Has anyone got a form that they would be willing to share that will allow you to add problems from a list box to the patients problem list?

The attached form adds problems to the problems list and/or to an obsterm for eye specific problems. The forms uses checkboxes instead of listboxes. Hope this helps.

Oops...that one only goes to problem list. This one goes to problem list and/or obs term.
