Does anyone have a form for recording the results of a spirometer test/evaluation.

If you are using Midmark I think they have an encounter form, but I believe you have to request it. I don't know if that would meet your needs. We don't use their encounter form, but that's what I remember reading in their documentation.

We have been using the Midmark IQmanager® for awhile now. I just called Midmark and it looks in our scenario it would be a change of software and a cost involved for moving over to the encounter. Having a sales rep follow up with me to see if it is a cost efficient move for us.
