We are looking to incorporate CCM in our practice. Is there add-on products to track this without having to manually track this (spreadsheets)?

Tracking would include:
Documenting patients approval of signing up for the CCM program
Documenting monthly phone notes with time included
Billing monthly for the CPT codes and tracking

Heres what we use in combination for that. Then use a crystal report for reporting. I'll see if I can get you that file.
