Hello,
Does anyone know what is best practice for how long a physician should keep a note open? Our doctors say it takes them 3-5 days or maybe even longer to complete and sign an encounter. That seems way too long to me. I tried to Google this topic to find a creditable source on best practice for completing an office note but I cannot find one. If anyone know a source I can review and present to the organization that would be great!

That, I am not sure. I asked our Meaningful Use expert and she said she thought is was a Joint Commission rule, not MU.
