We have recently brought up a Urologist in our group and he is finding the PE GU form is lacking in some basic items. We are checking to see if anyone has any Urology forms they would be willing to share with our clinic!

Prefer VFE, but if HTML available we can use as well.

I cannot help you with VFE form. We (mdObject) build only HTML forms. Unfortunately, I cannot "share" anything that we built for other customers and get paid. However, I will be happy to give you free guidance. What specifically are you looking for? ("prostate cancer", "adult patient history", ...)
