Pulling allergies into a form

We are having trouble pulling allergies into a custom form we are building. Anyone know the path for that or have the code?

Katie, open the EMR help and on the Contents tab, expand Using data symbols, Symbols listed by category and then Allergies. There are 9 MEL symbols that deal with Allergies.
