My role is changing, and it's not likely that I will continue to develop Centricity forms, so I thought I'd post some I have done that I'm particularly proud of. Feel free to use, change, tweak, modify, or ignore.
I am including both the "Pre-Op Assessment" form, and the text component that is pulled into the chart note by the button at the end. You will want to upload both before you test it out.
